GLP-1 Drug Interactions: Semaglutide, Tirzepatide, and Oral Options

GLP-1 drug interactions are not identical across medicines or formulations. Semaglutide and tirzepatide can affect oral-drug absorption through delayed gastric emptying, but Foundayo, the oral small-molecule drug orforglipron, also has important enzyme- and transporter-related interactions.¹²³

Give the prescriber and pharmacist the actual brand, route, dose, and complete medication list. “I take a GLP-1” is not enough to apply the correct label. This guide identifies discussions to have; it is not a list of instructions to change other prescriptions yourself.

Insulin and sulfonylureas: review low-blood-sugar risk

Combining a GLP-1 medicine with insulin or an insulin secretagogue, such as a sulfonylurea, can increase hypoglycemia risk. The current Wegovy, Zepbound, and Foundayo labels call for attention to these combinations; clinicians may need to adjust the companion diabetes treatment.¹²³

Ask what monitoring is appropriate and what symptoms or readings should trigger contact. Do not reduce insulin or another medicine independently. If eating becomes difficult because of digestive symptoms, tell the team, since a prescription plan needs to fit your actual intake and condition.

Oral contraception: do not apply one rule to every GLP-1

Zepbound labeling advises changing to a non-oral contraceptive or adding a barrier method for four weeks after starting and four weeks after each dose increase. Foundayo has a corresponding precaution of 30 days after starting and each increase. Non-oral hormonal methods should not be affected by this absorption issue.¹³

Wegovy does not carry the same routine backup-method instruction. Its label reports no clinically significant change in the studied oral contraceptive ingredients' pharmacokinetics.² That does not resolve every pregnancy-prevention question, particularly when vomiting, diarrhea, or missed pills occur. Discuss those circumstances with the contraceptive prescriber or pharmacist.

Oral semaglutide: administration timing matters

Wegovy tablets have instructions to wait at least 30 minutes after taking the tablet before food, beverages, or other oral medicines. A person taking another time-sensitive morning medicine needs a workable schedule reviewed by the pharmacist, not an assumption that swallowing everything together is equivalent.²

In the semaglutide-tablet interaction study cited in the label, levothyroxine exposure increased by 33%. This is a study-specific exposure finding, not proof that every injection user needs a one-third change in thyroid medication. The label recommends considering additional clinical or laboratory monitoring for oral drugs needing close monitoring or having a narrow therapeutic index.²

Ask how to coordinate thyroid testing and administration. Do not convert the percentage into a self-directed dose adjustment. The oral-treatment comparison distinguishes products that share the convenience of a tablet but have different instructions.

Tirzepatide and other closely monitored oral drugs

Zepbound calls for monitoring oral medicines whose effectiveness depends on threshold concentrations or that have a narrow therapeutic index, with warfarin given as an example.¹ The appropriate plan depends on the drug and clinical circumstances; it is not a claim that every swallowed medicine becomes ineffective.

Foundayo has additional interactions

Foundayo is not simply injectable semaglutide in pill form. Its label restricts combinations with strong CYP3A4 inducers and strong CYP3A4 inhibitors that also inhibit OATP1B, such as ritonavir. Other strong CYP3A4 inhibitors require a lower Foundayo maximum; moderate inducers require assessment of effectiveness. Its label also limits concomitant simvastatin to 20 mg daily.³

These are prescribing constraints for the clinician and pharmacist to apply. Do not stop an antiviral, seizure medicine, or statin because an internet list suggests a conflict. Ask them to reconcile the combination and alternatives. Separating tablets by a few hours does not automatically solve enzyme-related interactions.

Other treatments and procedures belong in the review

Do not add another GLP-1 or a second product containing the same molecule without the prescriber reviewing the existing treatment; current labels advise against relevant overlapping use.¹²³ Include compounded products, supplements, and medicines purchased elsewhere.
