Zepbound vs Wegovy: Weight Loss, Cost, and Which Fits
Compare Zepbound and Wegovy using head-to-head research, current treatment choices, and the costs that matter after the first prescription.
Zepbound produced more average weight loss than Wegovy 2.4 mg in the SURMOUNT-5 head-to-head trial: 20.2% versus 13.7% at 72 weeks. That result helps answer the effectiveness question. It does not settle which treatment you can afford, tolerate, or get covered.1

What the comparison actually tested
SURMOUNT-5 enrolled adults with obesity without diabetes and compared maximum tolerated weekly tirzepatide doses of 10 or 15 mg with semaglutide doses of 1.7 or 2.4 mg. It was an open-label study, funded by Lilly. Its strength is that the two treatments were compared within the same trial rather than across separate populations.1
If you start at 220 pounds, the trial averages correspond to roughly 44 pounds versus 30 pounds. That is an illustration of the percentages, not a prediction of your result. Some people lost more, some less, and some stopped treatment.
The choices in October 2026
| Question | Zepbound | Wegovy |
|---|---|---|
| Active ingredient | Tirzepatide | Semaglutide |
| Usual dosing rhythm | Weekly injection | Weekly injection or daily tablet |
| Trial behind the direct comparison | SURMOUNT-5 | SURMOUNT-5, injection up to 2.4 mg |
| Important additional indication | Moderate-to-severe obstructive sleep apnea in adults with obesity | Cardiovascular risk reduction in eligible adults |
| Higher-dose consideration | Discuss approved maintenance doses with your clinician | Wegovy HD 7.2 mg was not tested in SURMOUNT-5 |
The current prescribing information includes Wegovy HD and tablets. Neither can inherit the exact head-to-head conclusion for the 2.4 mg injection. A comparison using separate trials is useful context, but it remains an indirect comparison.23
For the wider molecule question, read tirzepatide versus semaglutide. If swallowing a tablet matters more than a weekly injection, compare Foundayo with Wegovy pill and Zepbound.
Compare the bill you will keep paying
Get three numbers before choosing: the pharmacy charge at your expected maintenance dose, the prescriber's recurring fee, and your cost if insurance stops paying. A low starting-dose price is a poor proxy for a year of treatment.
Lilly's self-pay offer starts at $299 for a 28-day supply of Zepbound vials or KwikPen; higher doses have different prices and refill conditions. NovoCare's Wegovy injection offer has its own introductory terms and maintenance prices.45 Our device and price guide explains why the Zepbound delivery format changes the shopping comparison.
Do not divide a posted drug price by a trial's percentage loss and call the result better value. That calculation ignores treatment duration, discontinuation, medical benefits, and individual response. Instead, use the cost calculator to compare expenses over the same period.
Coverage can decide before effectiveness does
Ask your insurer about the specific product, indication, dose, pharmacy, and prior authorization requirements. Being eligible medically does not mean the drug is included in your benefits. A medication covered for a separate approved indication may follow different rules from weight management.
Medicare members should read the GLP-1 Bridge guide, then ask their clinician which coverage route applies. Do not switch a medication that your plan already covers simply because another program advertises a lower copay.
Choosing a prescribing service
A useful provider explains how it handles side effects, interruptions, coverage denials, and dose changes before taking payment. Ro's review covers the distinction between care fees and medication access; NovoCare Pharmacy's review explains the manufacturer pharmacy route. Compare further options on the semaglutide board and tirzepatide board.
Bring your priorities to the appointment: the amount of weight loss you hope for, other health conditions, whether you can sustain the bill, and how you feel about injections or daily dosing. The right choice is one you and your clinician can sustain safely, not simply the drug with the largest trial average.
To put a trial average in your own units, use the weight loss percentage calculator. It illustrates arithmetic, not an expected result. If you are considering future drugs, our retatrutide evidence guide separates study results from approval and access.

Frequently asked questions
Does Zepbound cause more weight loss than Wegovy?
In SURMOUNT-5, tirzepatide produced 20.2% average loss versus 13.7% with semaglutide up to 2.4 mg at 72 weeks. The trial did not test Wegovy HD or tablets.
Can I switch between Zepbound and Wegovy?
A clinician can assess switching and the restart dose. Do not overlap them or convert doses yourself.
