The GLP-1 cost calculator — what you pay at month twelve, not what the sticker says.
Advertised monthly rates leave two things out: intro prices that step up, and terms you cannot leave early. Both are recorded in the prices these providers filed, and nothing on the internet computes with them. This does, across all 539 providers we rule on.
What these totals cover — and what they leave out
Medication program only. Every figure is the price each provider publishes for the medicine, extended over your horizon with its own conditions applied. Our ranking does not record consultation fees, membership fees, labs or shipping, so this calculator does not print them — an invented zero would be as false as an invented charge. Treat every total as a floor for the medication line, and ask any provider you shortlist what else lands on the first invoice.

Multiply the advertised monthly figure by 12 and you get one number. Follow the terms the provider actually filed and you get another. Across these rows the difference totals $2,200 over 12 months. Two mechanisms cause all of it: a rate that steps up after an intro period, and a term you cannot leave early.

The two mechanisms that make a headline rate wrong
Neither is a secret and neither is illegal. Both are simply absent from the number every price ranking — including, until now, ours — sorts on.
Mechanism one
The rate steps up
A provider quotes an introductory month and a standard rate thereafter. A ranking that reads the first dollar figure in the string ranks it on the introductory month — a price you pay exactly once. The largest example in our ranking is Peter MD, whose filed tirzepatide price steps from $149 to $249 after the first month. Twelve months of the headline figure implies $1,788. The schedule bills $2,888 — a gap of $1,100.
Mechanism two
The term does not end when you do
Several of the lowest rates in our ranking exist only behind a twelve-month commitment, and a few behind a quarterly or six-month one. The rate is real. What is not disclosed by the rate is that stopping in month three still costs a full year. Set the horizon above to 3 months and every committed plan jumps to its full term — which is exactly what happens on the invoice.
There is a third thing a headline rate hides that this calculator deliberately does not model: dose tiering. A provider may quote one rate at a starting dose and another at a maintenance dose. Our ranking records one published figure per provider per molecule, so we cannot compute a tiered price and will not guess one — the honest move is to tell you to ask. The titration planner shows how far your dose travels, which is the reason to ask.
How this differs from our cash ruling
Two pages, two different questions, and it is worth being explicit about which answers which.
Best GLP-1 without insurance ranks providers on the lowest figure each one advertises, and separately on the lowest figure with no condition attached. It is a ruling: it names a winner and defends it. This calculator asks a narrower question with your own horizon in it — what will I have paid by then — and answers it with arithmetic rather than judgment. Where they disagree, the ruling is telling you who to trust and the calculator is telling you what to budget. Both numbers come from the same 539 roster records.
Questions this calculator gets asked
- How much does compounded semaglutide cost per month?
- Across the 387 providers in our ranking that publish a semaglutide figure and are not a microdose program, the cheapest twelve-month total works out to $59 a month effective and the most expensive to $1,999. That is a $23,280 difference over a year for the same molecule. "Effective" matters: it is the total billed divided by the months you are on the hook for, which is not the same as the advertised monthly rate wherever a term commitment or an intro step-up is involved.
- Why is the total higher than the monthly price times twelve?
- Two reasons, and this calculator separates them. First, intro rates: a provider may advertise a first-month rate and bill a higher one from month two, so the number in the headline is a price you pay once. Second, term commitments: a rate offered "annual" or on a twelve-month plan is real, but you buy all twelve months whether or not you want them, so leaving in month three costs a full year. Across both molecules those two mechanisms account for $2,200 of billing that no headline rate in our ranking discloses.
- Is compounded always cheaper than brand-name Wegovy or Zepbound?
- No, and that claim is out of date. Both manufacturers now sell direct to self-pay patients at rates that overlap the compounded range, so the cheapest compounded program is not automatically cheaper than the cheapest brand route — and brand is an FDA-approved product where compounded is not. Our ranking records cash prices and a brand-access flag, not brand self-pay pricing, so this calculator compares compounded programs to each other and says so rather than implying it has ruled on the brand question.
- Does this include consultation fees, labs and shipping?
- No, and it will not pretend to. It prices the medication program at the figure each provider publishes. Our ranking does not record consultation fees, membership fees, lab charges or shipping, so putting a zero in for them would be an invented number dressed as a complete one. Treat every total here as a floor for the medication line only, and ask any provider you are seriously considering what else appears on the first invoice.
- Why are microdose programs excluded by default?
- Because a microdose is not a labeled therapeutic dose, and the cheapest figure on our whole roster is one. Ranking it against full-dose programs would put a different product at the top of a ranking people read as a like-for-like price comparison. The toggle turns it back on, clearly labeled, for readers who genuinely want to price that route.
- Do you earn money from these rankings?
- From some of them. The ranking is sorted on cost, honestly — several of the cheapest rows pay us nothing at all, and they still sort above providers that do pay us. We would rather rank on the number the page claims to rank on than quietly reorder it, because a cost calculator that is not sorted by cost is useless for the one job it exists to do.
